1. You will have to pay a non-refundable application fee.
2. An application Form DS-156, completed and signed. These forms are free at all U.S. consular offices.
3. 1 photograph 1 and 1/2 inches square for each applicant, showing full face, without head covering, against a light background.
4. For the "F" applicant, a Form I-20A-B. For the "M" applicant, a Form I-20M-N.
5. You will need to show that you have enough money to support yourself.
